To get the forms you can either:

- Or Collect them from the Smart Nation Department
You must submit the form or sent it by email, post or fax to the Smart Nation Department, preferably one week prior to the installation of the network.
The Department will then contact you, if necessary, to change one or more requested frequencies which cannot be allocated. Following approval, you will be sent an authorisation and the related tax payable.
